Group Coordinators Please Note:
The travelers' names that you enter here will ultimately be used to make travel reservations. Please double check all entries that you make! Name changes, if the airlines allow them, often incur heavy penalties and you will be responsible for payment of those penalties. Also, the name of each traveler MUST match the name on their passport. No nicknames are allowed.
 

Instructions:
In order to assure accuracy, you will need to enter the name of each member of your group twice on the same line (one group member per line). Enter the name once in the left-hand 3 columns and then re-enter it again in the next 3 columns. If there is any difference between the two entries a red "L", "F", or "M" will appear telling you that the Last, First, or Middle names do not match on that row. The form will not submit if any names don't match.

If you need more lines to complete your group, click the "Add 10 More Rows" button. When you are ready to submit your list, click the "Submit" button and the list will be forwarded to City Express Travel with a "cc" to you by email (you must include your correct email address in the field marked "Your Email").

Validation Exceptions: Since the form provides blank lines that do not require completion, blank fields are NOT validated. If you enter a name or initial on one side of the form but not the other the validatior will NOT pick it up and flag it for you. Please be sure that entries are made in both sides of the form in the same fields.
